First, choose the ESA Letter Consultation/Evaluation option, answer the required questions on the Intake Screening, and submit your request. The information you enter is secure and confidential. Once the screening results are reviewed, we will contact you within 48 hours to confirm the appointment. Completing this screening form does not establish a patient-provider relationship. Also, if the appointment request needs to be rescheduled for another date or time, or has to be declined, you will be notified via email or phone call, as well. After your appointment is scheduled, you will receive a link to register for the Patient Portal so that consent forms and questionnaires can be completed at least 24 hours prior to your scheduled appointment. Payment for your visit needs to be paid in full prior to the scheduled appointment. After the ESA consultation/evaluation is completed and the ESA Letter is approved, you will receive the letter via the Portal account and/or via your email. Once you enter your Patient Portal, you will see a notification for documents, consents, or questionnaires that are waiting to be completed.
